Following are the forms that Sheltie Rescue of Central Indiana uses in it's daily activities. These are provided in Adobe Acrobat 6.0 format so that you can open and print them. Please remember we adopt only to Indiana residents or those living near Indiana.

Please choose one of the following: I am a resident of Indiana and would like to obtain an Online Application to Adopt I am a resident of Indiana and would like to obtain a printable Application to Adopt I am not a resident of Indiana but I would like to apply for adoption

• Foster Application -- signature required

• Member Application -- please include your membership fee

• Sponsor-A-Sheltie Form -- be sure to include your donation

• Volunteer Interest Form

• Submit A Sheltie (Online Form)  -- please use this form in the event that you live in the state of Indiana and need to surrender a Sheltie to our group. Or you may call Nancy Cassel at 317-271-3848 or John Mountford at 765-342-5885 for more information.
